Privacy policy

Undivided Community Support (ABN 18 086 680 620) is bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and by the Health Records Act 2001 (Vic). This policy explains what is collected, why, and what you can do about it. If anything here is unclear, ask and it will be explained in plain language, in English, Turkish or Portuguese.

What is collected

Only what is needed to provide support safely and to invoice correctly. That usually means your name and contact details, your NDIS number and plan management arrangement, your goals, and health or disability information relevant to your support. It also includes case notes recording what was worked on in each session.

If you send an enquiry through this website, the form collects your name, your contact details, your funding type and whatever you choose to write. That is why the form asks you not to go into detail at that stage.

Why it is collected

To deliver your support, to keep you and your worker safe, to write the case notes your plan review depends on, to invoice you or your plan manager, and to meet record keeping obligations under NDIS and tax law.

Who it is shared with

Your plan manager or the NDIA, where they pay your invoices. Your support coordinator, school or clinicians, but only where you have asked for that. Nobody else, unless the law requires it or there is a serious and imminent risk to someone's life, health or safety. Where that happens, you will be told wherever it is safe and possible to do so.

Your information is never sold, never used for marketing, and never shared with overseas recipients.

How it is held

Records are held in access controlled systems protected by two step verification. Only workers who need a record to do their job can reach it. Paper is avoided. Records are kept for seven years after support ends, as required, and then destroyed.
